Cenote Diving paperwork and forms

All agencies and diving operators require that you fill out your paperwork before you begin the course, and this always takes a little time, as there are quite a few forms. This is your chance to read the paperwork properly, print off and sign. If you have any questions, just ask us via any of our contact routes.
This form needs to be printed out and filled in to protect the training agency from any liability. The agency we use is the IANTD. There are many other agencies that have become established in all parts of the cave diving world, but we use the IANTD. This is purely because the IANTD have an active presence here in Quintana Roo, and we can have your certification card delivered to you before you leave. You can’t get that with other agencies in this area. All respected technical cave courses have the same content, and it is your instructor which is important, not the agency. Underworld Adventures always over compensates on all standards, such as making the minimum amount of time for a cave course 8 days, and not the 7 days which is standard.
This form needs to be filled out to protect Underworld Adventures and their instructors, Dai Jones or others from liability.
This form is required so we can contact your family and friends should you need us to do so.
This is a standard form which all agencies require. If you tick “yes” to any of the questions, then you will need a medical waiver, which you can get from your doctor. Underworld Adventures reserve the right to over ride your physicians decision, and therefore, if you feel there is an issue, you will need to discuss it with us before hand. We also use this form to put your personal insurance details, such as DAN.
During a course, you will need to log all your dives, as a record of your training. There is room to log two dives per sheet, and, depending on the course, you will need between 4 and 12 sheets with you, which when completed, go into your file on your permanent record. We try and complete these at the end of each day
Before you arrive, it saves a lot of time if you can read and fill out the relevant paperwork, which will be discussed with you before you start the course.
